Healthy Together: Grounded in Wellness
Healthy Together: Grounded in Wellness is a three-month community wellness program.
This program was formerly known as Movin' Mountains. It has been updated and relaunched to better serve the entire community and provide more ways to participate.
The 2026 challenge runs from March 1 through May 31.
Participants may choose to:
• Complete a free monthly bingo card
• Join the $10 monthly challenge to track progress and compete for prizes
Each month has a different focus:
• March: Nutrition
• April: Active Movement
• May: Whole Health
Registration forms and program materials are available in English and Spanish.
How to Participate
Participation options are listed below. You may join one or both.
Free Monthly Bingo
Use the bingo card to track wellness activities throughout the month.
Prize Distribution (per month
• Complete one full row, column, or diagonal to earn one raffle entry (up to 4 entries per card).
• Three raffle winners are drawn each month.
• Complete the entire card (blackout) to receive a guaranteed prize instead of raffle entries.
Eligibility Requirements
• Completed bingo cards must be submitted by the monthly deadline.
• Submissions must be received before winners are announced.
• Each participant may receive up to two prizes per month.
$10 Monthly Challenge (Cash Prize Competition)
Participants track wellness activities using the monthly log sheet.
Points are earned throughout the month based on completed activities.
Prize Distribution (per month)
• First Place – 50% of the total prize pool
• Second Place – 30%
• Third Place – 20%
Winners are determined by total points earned during the month.
In the event of a tie, winners will be selected by raffle and the prize amount will be divided evenly.

• Registration and payment must be received and confirmed before prizes are awarded.
• Logs must be submitted by the monthly deadline.

Payment Information
Free Monthly Bingo: Registration is required. No payment is needed.
$10 Monthly Challenge: Registration and payment are required.
To qualify for prizes, payment must be received and confirmed by Jefferson County Public Health. Payments are verified before monthly winners are announced.
Late registrations and payments will be accepted. However, participants who register late will have less time to earn points and compete for prizes.
If you choose the $10 monthly challenge, payment must be made at the time of registration.

The Warm Springs Health and Wellness Center is available to provide information and printed materials only. Registration forms may be picked up there, but payment must be completed through Jefferson County Public Health.

Participants are encouraged to take part in local wellness events throughout the program.

2026 Health Fair at the Madras Aquatic Center (MAC)
• Wednesday, April 22, from 10:00 a.m. to noon at the MAC (1195 SE Kemper Way)
